RENTAL PROCESS

At Simplex Onsite (SOS) we strive to make your rental experience efficient and hassle-free. For new customers, the rental process can be completed in four easy steps:

  • Provide SOS a copy of your insurance certificate (example below).
  • Select a payment method (listed below) and complete the associated paperwork (forms below).
  • Complete and return a W9 (form below).
  • Sign and return the rental agreement provided by your SOS sales representative (example below).

Customers that already have an account relationship with SOS need only sign and return the rental agreement.

METHODS OF PAYMENT

Rental payments can be made via open account, credit card, check or wire transfer.

If you would like to establish an open account with SOS, please complete and return the credit application and accounts payable contact information below. While our standard payment terms for accounts with good credit and a good payment history are Net 30, we carefully evaluate each application on its individual merits. When the evaluation is complete, we’ll let you know what payment terms we can extend to you.

If you would like to pay by credit card, please complete the credit card authorization form (below) and return to your SOS representative.

Checks should me made payable to Simplex Onsite, Inc.

If you have special payment requirements, let your SOS representative know and we’ll make every effort to accommodate your needs.
